First Lease Option Deal In Reach - Memorandum of Option Form

Good Morning All,

I'm in the midst of my first lease option deal and I was wondering if anyone can tell me how to properly fill out the Memorandum of Option form so that I can publish my claim in the subject property? 

Also, any other tips or suggestions regarding lease options would be great!
