Questions for previous landlord during Tenant Screening?

Does anyone have a list of questions to ask a potential tenant's previous landlord during tenant screening? When I've called previous landlord, many of them have required me to have a written list of questions and are unwilling to provide a "general assessment."

1.) Was the rent paid on time? Late how many times?

2.) Was the apartment left in good condition?

Any others? I'm just hoping for an existing list so I can get good information from the previous landlord.

Here's some questions I often ask:

- What dates did the applicant(s) reside at your property?

- What is/was their monthly rent amount?

- Did the applicant(s) ever pay rent late or miss any payments? If so, how many times and when?

- Did the applicant(s) ever have any checks returned due to non-sufficient funds (NSF)? If so, how many times and when?

- Does this applicant(s) currently owe you any money for any reason? If so, how much and for what?

- Any complaints received regarding this tenant or their unit?

- Was the applicant(s) ever served a 3-day notice, eviction notice, or asked to move? If so, what was the result?

- If you had the opportunity, would you rent to this applicant(s) again?

- On a scale of 1-10 (10 being the best) how would you rate this applicant(s)?
