Subletting Process and Agreement

I have a tenant who is going to Europe for 4 months, who want to sublet the rental property during that time. She wants to continue to live at the rental when she returns, and she a great tenant - so I am going to let her sublet. 

Does anyone have a process in place for subletting?

Does anyone have a subletting agreement they can share?
