Looking for an Assisting Living Company to lease my house to

Where can I find local assisting living companies who lease or rent homes from landlords? 

I have a property with 6 rooms and plenty of space to make it into an assisted living facility. First time looking into this part of leasing. 

I would preferably like a company who would have all certifications and run the facility themselves. 

Any guidance on this topic would be greatly appreciated thanks!
