Tenant moved out, owes rent and lease breaking fee but has disappeared

I manage a townhouse in the Atlanta GA area (Acworth GA) and my tenant moved out without a forwarding address.  She owes me 2 months rent and a lease breaking fee (another 2 months of rent).  I was hoping to sue her in small claims court but need an address so I can have the sheriff local to her serve her papers.....but she has disappeared and I'm not sure how to find her.  I do have a copy of her drivers license and social security number from her rental application, but she didn't set up a forwarding mailing address (I sent a certified letter and it just came back to my property).  She does not answer/respond to calls or text messages on her phone.  Any help or ideas of how to find her / reach her to get my late rent / fees is much appreciated.
