NEW to BP- Help Filling out Chicago Rental Lease agreement

Hi all,
I'm a pro member and just downloaded the IL lease package. I'm not sure how to answer a few of the fields in the Chicago lease agreement. Does someone have it already filled out they'd be willing to share it with me, please? For example, section 1 a) do I put my name or do I define that the landlord is also the property owner? 

It also seems to be missing all of the disclosure documents.. Does anyone know where I could get those?
