End of Lease - Termination Letter

Hi, 

I noticed that Bigger Pockets doesn't have a termination letter template and I have a rental in VT (Windsor County)  that is coming upon it's second year ending.  The tenant pays on time, but I recently did a walk through and she is violating many health/fire codes due to what appears to be a hoarding situation that has grown over time.  I do not have interest in renewing her lease and want to send a termination letter.  Would anyone be able to help identify what should be written in this letter for clarity and is it necessary in every state regardless that the lease says it terminates at end date unless renewal lease is signed, etc.

Any insight would be much appreciated,
