Sign Off Form re: Repairs

Does anybody recommend a sign-off form I should provide to Tenants after I had major repairs done from a shower valve leak ?(ruined an entire bathroom, and some adjoining flooring).  (I paid out of pocket, as insurance refused to cover, to a very reputable contractor, after water mitigation was done). . I was going to write something to the effect “Tenants acknowledge all repairs were completed so that property is once again satisfactorily habitable.”

where are you located? Nothing like that would be required in Ohio. If a tenant is unhappy with a repair and I thought the complaint was unreasonable, I would just tell them that the fix is satifactory to me. If they persisted with the complaint, I'd probably let them out of their lease if they wanted to move (ie 'happy clause' them)... Since you're not happy here...
