Philadelphia rental license

Hi, I'm trying to renew my rental license for a Philadelphia duplex that I also live in. And the eclipse website is asking me to get a notarized affidavit stating that one unit is being occupied by me. I just started being a landlord 2yrs ago and I don't remember having to do that when I renewed the rental license last year. 

Is this a new requirement or am I missing something?

Also do I need to get a new certificate of rental suitability everytime I renew a lease?
