[california] ab1482 exemption forms?

Are there any boilerplate ab1482 exemption forms available online? 

I am renting rooms in my home and am therefore exempt from ab1482, since we share kitchen facilities. I think we are only exempt from the "just cause" part. In any case, our situation is not unique so I'm hoping someone knows of some forms available online somewhere?
