What does the actual SBA PPP documentation look like

To apply for the SBA PPP forgiveness, I would need federal and state forms, along with cancelled checks and a statement of truth, to qualify. Does anyone know what statement I will need to attest to, or how many quarters of fed/ state withholding statements need to be turned in. Does 2019 statements need to be included? Has anyone heard?
