Creating Personal Asset Statement

Hey all! Starting the process of getting a mortgage and I need an asset statement. Any advice or templates you could share with me? I've done personal budgets on Excel but not quite sure how to lay out or what all to include in an asset statement. 

How detailed does it need to be? For example, personal property, should I put camera gear and firearms as seperate line items or would those fall under a specific category (i.e recreational equipment)? Thanks all!
