FSBO - Medicaid Issues

We are in talks to buy a FSBO. The owner wants to sell but doesn't want to lose Medicaid because it's paying for their nursing home. Is there a way to structure this deal so that they don't lose coverage but we gain ownership? At the same time, there are some code violations & issues so they really don't want to hold onto the property much longer. Would Seller financing or rent-to-own help at all? Thanks!
