How to Update Operating Agreement

Changed my LLC address and updated that info with the state. Documented it with LLC Amendment Form and signed it. Question: Do I edit the actual LLC document to reflect the change, or do I simply attach the amendment form to the back of the Operating Agreement (and leave original LLC doc alone)? Thanks!
