Independent Contractor Agreement for BSOW Drivers

Hi everyone, I’m not a very active member on the forum yet but I am working on learning and absorbing as much info as I can right now. We have taken some action and have a direct mail campaign set up already and right now we are in the middle of launching our Bandit Sign On Wheels program but I want to make sure that we have an independent contractor agreement established for our drivers to read acknowledge and sign prior to us installing a decal on their vehicle.

If anyone could help us with making an agreement that we can reuse multiple times that would be much appreciated, or if someone could point me towards someone that I can reach out to that would also work.
