Renting a primary residence house - personal liability issues

I am putting my primary residence home up as a rental. Hoping for advice of the Pros:

1. What is the most cost-efficient way to transfer a property to LLC? Attorney, CPA, paralegal?

2. Does an LLC need to be maintained once it's created?

3. Is it worth the money to hire an attorney to go over the property management contract that I will be signing with PM?

4. Any other steps I need to take to protect my personal assets from liability and lawsuits?
