How to Send a 1099 Without an Address

I am in the process of filing taxes. As an owner of one rental property, I found out that I was/am supposed to send a 1099 to a contractor who did some work for us on our rental property last year, and file a 1099 on that same contractor with the federal government. Obviously I was completely unaware that this was a thing until now. As a result, I am late in sending these forms, and on top of that, the only contact information I have for this contractor is his phone number. I am a bit nervous to call him to ask for his address and TIN number, because I am so late getting to this. I don’t know if it will mess with his taxes if he has already done them, or if he will be willing to give me his information. 
I guess my question is, am I correct to say that my only course of action is to call this guy to ask for his information? And what do I do if he doesn’t answer or avoids giving me this info (I have read other people saying this was an issue for them)? Thanks.
