Repayment of 2008 First Time Home Buyer Tax Credit: HELP!

Last year we paid our first installment of our 2008 FTHBC. We took the credit in December of 2008. I lost my job in the spring of 2009. We decided in 2011 to file bankruptcy and we vacated the property in February of 2011. The home was repossessed by Wells Fargo Bank in the summer of 2012 and to this day, has never been sold. Last year the IRS deducted the $500 from our tax return. This year we attempted to the best of our ability to fill out the 5405 form; however, it doesn't seem to apply to our situation. I am unable to determine if we are exempt from paying, must pay back at the rate of $500 per year, or if we must pay the full amount back in one lump sum. We made no gain from the property and no longer own or live there. Any information would be very helpful. Thank you so much!
