Title and legal issues on paid off home

We bought a house 4 yrs ago, it was a foreclosure. We paid a title lawyer in full and got title insurance. We went to go do an equity loan to fix the house and we’re denied because when the previous dismissal of mortgage wasn’t properly submitted by the title lawyer to the county clerks office. Lawyer is basically writing us off at this point and gave us the run around. Told us title insurance company will fix it, they can’t we were told it had to be his office. He said he didn’t know what he missed so we went to the county clerks office and through hundreds of papers on our property to find out. He finally agrees to fix it over a month ago. Still isn’t done and he dodged our phone calls. When is it appropriate to take legal actions against the lawyer? Do we go and report him to the bar? Do we sue him? We had similar issues with him when closing on the house 4 years ago, should of been a quick sale cash up front and it took 4 1/2 months to close.
