Boilerplate Template for Private Money

Can someone here help me with template (document) to raise money. I just want to go over it as I plan to raise some private money from friend/family. I want to keep them at ease and would like to issue them a note so that they can also sleep better. Would appreciate if you can share your document so that I can read and see what all goes in it. thanks
