Does anyone have a template they could share that encompasses rental income & also income derived from my company(which also comes from buying and selling homes as a Realtor) I am trying to give a to a bank for a loan.

Most banks should have a blank PFS form they can provide to you. Some may prefer their own format, but probably not required. Ask the bank.
