LOOKING FOR REI PARTNERSHIP QUESTIONNAIRE

Hello All...I know there are many possibilities for this one, but does anyone have a REI Partnership Questionnaire used to vet potential family members, associates, etc. who say they need to create income for retirement and want to partner with me in investing in Real Estate? It needs to be basic and easy enough to follow for me to explain what their possible roles could be. Also, if you know of any videos on partnership that would be great...why not just Google it? Because you guys are the pros and I don't want to waste a lot of time going down rabbit holes LOL. Anything you can do to point me in the right direction would be great.
