Rental Forms to confirm rent to give to Seller

A couple of Months back I heard a podcast where the Guest was purchasing a multi unit property and gave the seller a Form to fill out. Brandon said the name of the form but I do not remember it and now I find myself in that spot. I have a Multi Unit under contract and I am looking for that Form. I must have heard 3 podcast this morning. Please help.  Thank You
