Title issue with home purchased from VA

I'm trying to help someone out with a title issue. Gotta think this has happened before.

This gentleman purchased a rental property about 20 years ago in South Carolina. The home was purchased directly from the VA as they had previously repoed the property. Countywide originated a new mortgage for the buyer at that time. However, the deed for the purchase listed Countrywide as the grantee (specifically 'Bankers Trust Co Of California'). The tax info also shows the owner this way. Of course the mortgage is recorded correctly with the real buyer listed.

Is there an easy way to correct this mistake, or should an attorney handle this? Thanks
