Building on an empty lot

Can anyone give me an overview of the process of building a house on an empty lot? Do most people hire a general contractor to do all of the work? Does the GM source out laying the foundation and getting utilities set up? Is it possible to do the general contracting yourself?

What kind of house are you building and what are you going to use the house for? If you are just looking to build an "average" single family home, many folks go with modular/pre-fab-type houses. Many areas have architects and GCs that specialize in those kind of homes. Maybe go from there?
