Recommendations for tenant survey

Having recently closed on a small apartment complex, I'm putting together a survey for the tenants. My goal is to a) get their background information to better understand my target market, b) learn more about potential maintenance issues (e.g. have you ever had frozen water pipes?), and c) identify areas for improvement in the units and on the grounds.

What would be some good questions to ask?

Not sure every tenant will answer but it is a great start.  If you are using property management  I would ask how much contact they had with them and for what reason ,  maintenance , dropping off checks as well as what they were happy / not happy with.  I wouldn't ask about frozen pipes I would be more general. Issues with water, electrical, parking...   Also for tenant info I did not read this as if you were re-screening rather you were collecting contact information, work location and maybe emergency contacts.  One question that was helpful vof me was to ask was if there was anything special they wanted to mention and one tenant did add that they worked nights.  This was really helpful to know so I tried to respect that if there was maintenance scheduled.
