How often should I be doing home inspections?

As a landlord, how often should I be doing inspections on my home? Also, what exactly should I be looking for, things to repair or fix... For HVAC Filters, how often should those be changed, and is that the tenant's responsibility? I have it in my lease that they're supposed to maintain them, which I doubt any of my tenants ever do.
