Realtor Disclosure Question

I'm looking for some guidance from realtors that also wholesale or purchase properties themselves. My business partner is a realtor. We have an opportunity to do a cash purchase below market. The seller understands the deal is under market. Seller wants a quick sale. My partner is not the seller's agent. The property is not listed. But the seller knows my partner is a realtor. My partner has been the primary point of contact. We want to present the seller with a disclosure that covers this situation. Can anyone share what they use?
