seller interest disclosure form

I'm looking for a well worded form to disclose that I'm representing myself as the selling agent.  I sale my own properties and usually just add in a handwritten addition to the contract  stating this. I would like a separate disclosure form for this and my broker I work under doesn't have one.  Hoping some of you might have one.
