Finding decision makers

Over the years I have taught myself how to find the decision makers for property with the help of  the internet and tools such as TLOXP.  

Where I can find almost any decision maker I am having a hard time locating someone with a particularly desirable property. The property has been uninhabited for years. The owner is a senior citizen and I suspect is in a nursing home. However, There have been no liens recorded by masshealth yet to confirm this suspicion. There is a small heloc on the property from years ago that is headed to foreclosure. I would love to figure out who owns that note but all my attempts to find that person or entity have been futile. 

TLOXP tells me the owner has no relatives or next of kin.  His mail doesn't appear to be forwarded anywhere because the mailbox is always over flowing. 

Does anyone have experience finding and reaching out to people who are in nursing homes?
