Monthly cash flow vs. Cap rate % vs. Cash-on-cash returns

I just ran some numbers and found a property that has a cash flow of $273/month. BUT my cap rate percentage and COCR is terrible! Why? I am terrible at math and hoping to get some insight on this. Thank You!
