I feel horrible that this is my first post - I'm sure one of you can help me out, or at least I hope...

I'm in need of a proper form to submit an offer on a residential property in NYC. Is there anyone out there that could possibly help me out and send me the proper form?? THANK YOU. I hope to return the favor and also become more active in PB! Such an incredible site!

It's very simple. It's called a letter of intent and it is just that: a letter stating your offer and the terms of your offer. Include purchase price, closing date, whether you are financing, and any other contingencies, such as clear title.
Be sure to include language that the letter is an offer only, and that if the seller agrees to same, a formal contract will be required.
