Name of the form or data that provides property income & expenses

Should be an easy question for you.  I forgot the name of the form or data sheet that provides the list of rent payments and expenses (repairs, etc) that you ask for when you look at purchasing a rental property.  Usually goes back 2 or 3 years, at least that is what I have seen.  Can someone give me the name of this?
