Forms/checklists for meeting with sellers

Hey, all.

In the interest of purchasing foreclosures and meeting with sellers, I'm looking for some forms other investors may have that serve as sort of a preliminary checklist. I figured I would check on the forums before I run off to create my own form.

Here is the general idea of what I'm looking for. When meeting with a potential seller, whether it be somebody in pre-foreclosure or not, I want to have a form/checklist to keep track of information such as current mortgage info, tax liens, legal fees associated with the property, etc., along with general info such as the seller's info and home info.

Does anybody out there have such a form or something close to it? I'd love to see what you have and either use it or build off of it.
