So confused :-(…debt-to-income…???

I am confused about debt-to-income…any clarification would be greatly appreciated. When buying up rentals, how are you accumulating all these properties??…how are banks approving loans on so many properties?…wouldn't this throw your debt-to-income ratio way off..??? Please help.
