What do you think of my goal board?

Although I'm not much of a New Years resolution type guy, it coincidentally happened that it was time for my business partner and myself to make a new goal board. I thought it would be a great chance to share idea's on how others keep track of their goals and I would love feedback on the one I created!

We set this up with a 12 month goal, but it's broken down into a smaller 12 week goal. Our lead measures are what we believe to be the things that are the most directly correlated with our 12 week goal. We keep track of our 12 week goal with a point system and our 12 month goal is a bar graph that shows where we should be vs. where we are as we go forward. We also included a "To Do" section of smaller things that we had to take care of with dates to complete them.

Does anyone have any advice on tweaks I could make to the one that I created?? I would love to hear how others keep track of their progress.
