Concreted approach and driveway quote in Fort Worth TX

My rental home has a messed up approach to the driveway that was not properly patched by the previous owner and I didn't have the funds to fix it while I lived there. Now the approach has been scraping the bottom of my tenant's car and even took off a part of the car's bumper.

I was hoping to get some good quotes, but I'm not sure what is reasonable for a 690 sq. ft concrete driveway and approach. This would include removal of old concrete, #3 rebar, 5 inch deep concrete. The contractor said that he won't need to put crushed rocks because the ground is pretty compact from years of parked cars on the driveway.

my questions are:

1) is this a fair quote? Does this methodology sound right?

2) will my rental home insurance cover my tenant's damaged car?
