NYS Broker's license

I am applying for a NYS Broker's license and am wondering the following;

1. is it neccesary to fill out the experience/points sections on the application?  I was under the impression that is was either 2 years experience OR equivalent point total.

2. I own a property management company, should I apply for the license in the name of my LLC or in my individiual name? I may want to expand into brokerage under a different company name, so I assume it is best to apply under my own name?

Any help is appreciated.
