Single Family Ownership Split agreement

Hello, 

A friend and I have agreed to go in on a SF property to live in together for a few years.  We then plan on renting it out.  We will be contributing different amounts of up front equity but splitting the monthly expenses.  

Does anyone have an ownership agreement doc that you could share that I could amend slightly?   Looking to just get a template that we can work with.
