Administrative fee disclosure form?

Hello All,

I have finally went through the process of getting my first multifamily property and I close on Friday. Just a few moments ago my realtor sends me this administrative fee disclosure form. 

is anybody familiar with this and should I go through with it? is it mandatory that I sign?

I appreciate all the help given. thanks.
