How to Structure Investment Agreement

We have a group of 6 Newbies coming together to Fix & Flip a property. One person will be doing Construction Management, another will be handling the financials, the rest will only be contributing cash. All 6 of us are contributing various amounts of money but we will still need a lender. 

My questions are:

How should we structure a written agreement that is fair to everyone based on their sweat equity and financial contribution? How should we divide the profits? Is there a template somewhere that could help?

Should we get a mortgage under all of our names or just one/two?

This is my first post so I appreciate your help and guidance!
