Tips on transitioning a business from Sole Proprietor to LLC?

Hello,

I'm wondering what I can transfer over. F.e., I've got some cars that I rent out to people, and I also rent rooms our in the place I live. I'd like to transfer these activities to an LLC (that I make).

Some things, like a car loan, credit, etc, are currently under my name. Can I transfer these things to the new LLC that I make?
