Disclosure Forms For Wholsale Deals

Greetings BP Friends! I hope this isn't a silly question, but as a newbie, I wanted to know if there is a form that Realtors should use to disclose they the are in fact licensed, during a wholesale deal? I went through the forms here on BP and my personal forms and I want to make sure that I am using the right one. Or does it even matter, if I have an LLC strictly for REI. Any feedback is appreciated, thanks.
