Milwaukee Tenant / Landlord Responsibilities

Hi All,

I have all of the legal documents I need from https://www.wilegalblank.com/. It briefly goes over some tenant / landlord responsibilities, however I don't think it's very clear. Has anyone created a one-page document that goes over scenarios which lists which party is responsible? 

For example: The toilet is clogged (responsible for bill - tenant)

This is for very common maintenance calls, etc.

I want to make things very clear upfront so they know what's expected.
