Filling Out Contract for Duplex

I'd like to know if someone can tell me what if any differences there are when filling out the 20-12 1-4 contract (Texas) when purchasing a duplex. 

As far as I can tell the two units are listed as separate properties so on #2 of the contract how is this filled out? Should there be 2 separate contracts?

The property is actually from an agent so should I just let them fill it out? I'm kind of trying to avoid looking like a total newb here even though that's exactly what I am.

Help please! Wanting to make an offer tomorrow.
