Companies/Individuals that management residential assisted living

I’m having a hard time finding management companies that are basically the “property managers” of residential assisted living homes. 

Is this a thing or does everyone do this on their own?  

For example they would find/hire caregivers, find/place clients, collect pay, etc.  

I’d like to be the investor, but don’t know the ins and outs of finding, placing, and collecting.  

Do companies do this for a fee or percentage of gross revenue?
