Every time I see his company  name in print I cringe a little. 

Why not just print out a nice receipt with your name & address etc for total rents revieved 2016 and sign and date it.

Go to http://workcomp.virginia.gov/vwc and fill out form 61A-online. you can download it after filling it out so you can have a copy plus print it out for Norfolk license requirement.

I tried to search the forum, and it says there are results, but none are displayed.  

(when you use I in referring to yourself in print it is a capital letter as in "I will do that")  

Once I get the map printed ill update this post with the picture.

I guess for now I will continue to work off printed spread sheets and look to automated this process in the future. 

I'd be very wary, it might be lawyers or sharks that will get you with the fine print.  

Use it to seed your own process or as is - - print one per application, fill-out your findings, sign, date and attach to the original application.I operated a 6-plex and it took about 3-4 years to do this by rote, that's 24 successful screenings and maybe 3x that for reject/denials.Suggestion:  this is 20/20 hind sight now, but start tracking your 'good vs bad' eggs and correlate to the verification list results you filed away.
