I keep a blank one in my car for just these kinds of situations.  

Or you could get a blank copy from your attorney or a leasing agent and then just copy what you want from there.

On rare occasions, I get questioned whether I would put carpet in the bedrooms. 

Chances are much much better if you present your deal than to fill in the blanks and don't have your ducks in a row.
