Electrical outlet without ground wire, hot and neutral wires terminated at ground screws

As Jon said, most likely it's the neutral that terminates at the ground screw, and was wired that way to fool a standard electrical outlet tester.Either replace the outlet with a non-grounded two-prong or run a ground from another nearby outlet that is properly grounded.
