Find local senior care organizations, do enough googling or networking and you'll likely find networking groups where you can connect with folks in the biz then you can track down ownership or operators from there.

I would look into converting it into an actual duplex with separate utilities, but I have not had any trouble finding groups of 6 that all know each other.

Outside of the research you have noted, I'd suggest:- go to open houses and look at properties- find your local RE investment group (and join!)

The exception to this was a group of non communicative tenants, i inherited after taking over a property they didn't respond to any posted notices, phone calls or messages requesting information to resolve issues so that process moved pretty quick into eviction, but that's not the norm.
